QI Plan Evaluation

PHAB Standard 9.2.1 (A) requires a QI Plan that describes:

"The health department’s approach to how the quality improvement plan is monitored: data are collected and analyzed, progress reported toward achieving stated goals and objectives, and actions taken to make improvements based on progress reports and ongoing data monitoring and analysis."

Are you wiling to share a list of measures to monitor QI plan performance? Or are you willing to upload a sample QI Plan evaluation report?

I will be following this thread.  I'm currently working on trying to develop a process/format for exactly that.  I would be very interested it what others have done.

2016 was our first year with a QI Plan. We made some minor revisions to our plan for next year based on our experiences. We did not do a formal evaluation this year but I would at least like to do an Executive summary of accomplishments, lessons learned, etc.  During the initial plan writing phase we did the QI maturity survey (full version with the team and Executives and abbreviated version with entire staff). We also plotted our "phase" in the NACCHO QI roadmap.  We plan to go through this process to evaluate our progress in advancing our QI culture again in late 2017.   We also will use feedback from surveys of our QI project teams. Any examples of formal evaluation forms would be greatly appreciated!

Our quality council currently establishes 3-4 QI plan implementation goals annually with measurable objectives for each.  We use these to guide our activities through out the year and report on progress to the Executive Team quarterly on a dashboard (developed in an Excel spreadsheet-no fancy systems here).  We found the transition strategies in the NACCHO roadmap helpful in helping us establish our annual goals. Much of our 2016 goals were focused on establishing our infrastructure.  I attached a sample of our DRAFT 2017 dashboard.

Hi there!  Along with updating our QI work plan quarterly, I do these annual summaries for our staff, BOH, and other stakeholders.  They're easy to read and to the point.  We purchased a subscription to Piktochart and develop them using that tool.

In addition to our QI work plan that gets updated, we annually re-assess the portions of the (large) QI self-assessment that we chose as our focus for the year to determine if progress is being made.  You can also see that on the two pager QI summary.

Thanks for sharing these two resources, @jgoodern and @kwaksmonski!

@jgoodern, can you please share the QI team survey that you were referring to? I would like to incoporate that into our QI projects.

I have also attached a similar document to @jgoodern's showing how we minotor and evaluate our progress on our performance management and QI goals on a quarterly basis. This is an action plan for our agency's strategic plan but can also fit in with our QI plan.
