New PHQIX Drawing Begins!

Wednesday, September 14, 2016 - PHQIX is excited to announce the launch of our latest drawing! To enter the drawing to win one of our great prizes, submit a new QI initiative or update an existing in-progress initiative by November 1st, 2016. You must register on PHQIX to submit a QI initiative. By submitting your QI initiative on PHQIX, you will receive many benefits, including an AMA-style citation in phqix.org that you and others can reference. Your initiative can help inform similar initiatives being conducted by other PHQIX community members and contribute to the evolving science of public health quality improvement. CDC: National Preparedness Month Twitter Chat

New NQF Resource: Improving Population Health by Working with Communities: Action Guide 3.0

Wednesday, August 31, 2016 - Check out the National Quality Forum's (NQF) new resource, "Improving Population Health by Working with Communities: Action Guide 3.0." The Action Guide is a framework to help multi-sector groups work together to improve population health by addressing 10 interrelated elements for success and using the related resources as needed. Like a "how-to" manual, the Action Guide is organized by these 10 elements and contains definitions, recommendations, practical examples, and a range of resources to help communities achieve their shared goals and make lasting improvements in population health. It is intentionally brief and written in plain language to be as accessible as possible for all types of stakeholders at the local, state, regional, and national levels to take action. Webinar: Community Complex Care Response Team to Improve Geriatric Public Health Outcomes

Tuesday, August 16, 2016 - Are you planning or implementing an QI initiative to improve geriatric health outcomes? Join Systems for Action on Wednesday, August 24, 2016, from 12 to 1 p.m. EDT to learn more about this ongoing research project! This study evaluates Michigan’s Community Complex Care Response Team, a collaboration of three community agencies that provide services across the medical care, public health, and social and community services areas to decrease potential vulnerabilities and promote health, wellness, and independence in older adults. The research team will use a pragmatic randomized controlled trial to examine the impact of coordinated service delivery on emergency department utilization, explore which institutions are best positioned to perform integrator roles that connect vulnerable older adults to needed services and supports, and identify data sharing and storage challenges across health and human service sectors.
