Referral Management System

We are working on creating an effective referral management system for our department.  Please share your referral management plan, software support, QI processes, etc.

This past year we created a system (through SharPoint) and process to submit, track, and follow-up on referrals submited between programs in our health department. Are you interested in information about internal referrals, external referrals, the software, the procedure, etc.?

Most of our external referrals come via fax or phone. However, we too, use SharePoint with a couple different providers to exchange information with ongoing care/coordination of health information. We have used it for Measles testing and follow-up after an exposure in a local hospital as well as with an employee health department at a local hospital with staff/volunteers who test positive for TB and require LTBI treatment. SharePoint has been very useful in this regard.

Hi Jill,

We don't have sharepoint yet, but some of our programs (child health/perinatal case management) have benefitted from creating an excel document that is placed on our shared drive. The only negative about this is that multple users can't edit the document at the same time, but staff are aware of this and try not to keep the document open longer than they need.

One of our programs also uses OneDrive to store their excel document. OneDrive is nice because multiple users can open and update the document at the same time. This is still new so we are still learning how to use it.

Hi Jill, have you had any luck with creating a more efficient refferal management system? I've encountered many public health departments that could benefit from this type of discussion, and would appreciate hearing about your process.
