At the past NNPHI conference there was discussion of surveying the past participants involved in a completed QI project. Does anyone have a survey and/or questions that you have asked of your past teams to gather their feedback?
Hi Jill, are you refering to surveying QI project team members or individuals involved in the process being improved?
For project team members, I usually do a post-project survey via surveymonkey to ask them:
To rate their satisfaction with the project on a scale of 1-5
What they liked best
What they liked least
If their understanding of QI and QI methods has increased
Their willingness to be involved in another QI project
Other suggestions for QI projects
Additional comments for QI Coordinator
For individuals involved in the process being improved, I have done satisfaction surveys to gather baseline and post-implementation data. Here is a link to one for our package delivery process: https://www.surveymonkey.com/r/6N2PFBQ
Although I don't have any questions to add to Gurleen's great list, I will just add that I've used radar charts to display the responses to questions with Likert-scale responses. I think they provide a nice visual for a quick read of areas of strength and those areas needing improvement.
We ask past QI team members to rate the following using Strongly Disagree, Disagree, Agree, or Strongly Agree
My understanding of QI and QI methods/tools increased.
I had the resources necessary to participate in this project.
I had support from leadership to participate in this project.
Team meetings were effective.
I felt like I was able to contribute to the team.
I felt satisfied with this project and the QI process.
They can also answer the following questions if they have certain feedback for the QI Council.
1. What were the strengths of your team?
2. What challenges did you or your team encounter?
3. What advice or lessons learned would you have for future QI teams?
4. Additional comments for the Quality Improvement Council:
