Hello,
Does anyone have a risk management program policy you are willing to share? Through a program review, one of our clinics needs to strengthen their risk management program, and I'm curious as to what other folks include in their risk management program policies.

We also had a similar situation in the previous health department that I worked for. Since we had a QI plan and a QA process in place already, we created an "incident report" policy that described steps to report any risks identified or incidents that occured (in the clinic only). This incident report form was sent to the QA team, and then referred to the QI Council if it was worthy of a QI project. In retrospect, it might have been better to create an umbrella clinical risk management policy to encompass QA, QI, and incident reporting.
