Connecting QI Projects to PM and Health Outcomes

With PHAB's reaccreditation emphasis on performance management, quality improvement, and health outcomes, I was wondering how we could connect our QI projects more directly to our PM system and to health outcomes. Has anyone attempted this already, and if so, can you please share lessons learned?

Would it be approriate to list these as 2 additional details in the "plan" phase of the PDSA cycle?

Thanks for sharing, Channa! I agree that including QI and PM at the top level is important. Do you also do it from the bottom-up? For example, do you include in the QI project charter/storyboard how each project connects with the agency's strategic plan/PM system/ or CHIP/health outcomes?

Tacoma-Pierce County Health Department continues to share high-quality resources with the public health practice community. One resource is the 2017-2018 iteration of their  Quality Improvement Plan.

You asked how you might connect your QI projects more directly to your PM system and to health outcomes; Section 5 of the document discusses the relationship between quality improvement activities and the Performance Mangement System. Here's a sample from Section 5 that I think is relevant to your question: 

"The performance management system enables the department to focus on learning and improvement by integrating all aspects of management, policy-making processes and transforming practices and processes so the focus is on achieving improved results and better health outcomes."

Ty, the link you posted is not working when I click on it. Can you please try to reshare?

Based on the portion you pasted above, I like that statement as an umbrella statement to include in a QI/Strategic plan to encourage use of QI throughout the agency. I was hoping to get something more specific about connecting health outcomes to QI projects. Thank you for your help!
