Baseline assessment for performance management and quality improvement initatives

Is there a measurement tool that we can use to gather baseline data on staff's knowledge, willingness and/or readiness to participate in performance management and quality improvement initiatives, or a baseline to assess an organization's culture toward quality improvement?

What I am looking for is different than assessing whether we have a system in place which is assessed in the Public Health Foundation's Performance Management Self-Assessment tool. I am curious to learn more about perceptions of staff culture, accountability, knowledge, etc.

We gave a quality culture survey to our staff at an all-staff meeting prior to initiating our efforts to develop a quality culture, and we use those results as our baseline. We continue to give it at our all-staff meetings every 6 months to monitor our progress.  There are 16 questions which help us monitor staff's perceptions.  Staff score the questions using the following key:   0= No awareness or activity in this area, 1= Awareness and discussion, 2= implementation in some areas, 3= implementation across the organization, and 4= implementation and ongoing evaluation.  The survey is a modification of an assessment tool we found on-line, and I'd be happy to share it with our results.  Just email me Bartowr@stlouis-mo.gov

I would recommend using the tools available through the Roadmap to Quality Improvement.  There are a few assessment tools there.  Once you do the culture assessment, there is guidance on how to move each key ingredient to a quality culture forward!  A great resource.  Good luck!  http://qiroadmap.org/assess/

please see a new post from Rhode Island Dept. of Health about a survey tool to measure implementation of the NACCHO Roadmap.  this is a story you may wish to replicate in your health department (state, local, or Tribal)!
